Questionnaires

Questionnaires are Web based documents that can be used at GP, practice, and patient level. They are used to answers to any types of questions that are required. The completed questionnaires are stored centrally and can be viewed online.

The questions and answers can be exported as a CSV file.

Questionnaires are created using the Pathway Developer tool and distributed to practices using the Service Management Dashboard.

They can be defined to be completed within certain time frames, and then submitted by practices to provide data that can be viewed centrally.

Questionnaires fall into the following categories:

• Practice level. - Completed once by a single member of the Practice. Can be defined to be completed and submitted within a time frame
• Staff patient level.- Completed by members of staff against a list of patients. The designers of the questionnaire can email practices with a mail merge letter that invites patients into the practice to complete the questionnaire with a member of staff. Can be time frame specific
• Patient level. - Completed online by the patient on once.
